Parents and Guardians,
RF Athletics will offer athletic physicals on Thursday, May 14, 2026, at the Middle School Gym from 2:00–5:30 PM through Rockport Physical Medicine PA & Hawkins Chiropractic.
Cost: $30
Checks payable to: Rockport Physical Medicine PA
Current 6th and 8th grade athletes who turn in the following items to their athletic coach by Wednesday, May 13 will be eligible to complete their physical between 2:00–4:30 PM on May 14:
Completed Medical History Form
Completed Consent Form
$30 payment
Who needs a physical:
Students entering 7th grade
Students entering 9th grade
Please note: Athletic physicals are valid for two years.
